Pulmo Rehab / Breathwork/Homecare Patient/Caregiver
You are currently enrolled in a Pulmonary Rehabilitation or Homecare program.
1. Username: Enter the Email Address you provided during registration.
2. Password: Use the temporary password emailed to you, or your self-selected password. 3. First Time? Click "Register/Activate Account" below the sign-in button and follow the on-screen steps to set up your password and security questions.
Caregiver / Family Member
You are an authorized family member or individual providing support to an enrolled patient.
1. Username: Use the Email Address linked to your Caregiver/Proxy account.
2. Password: Enter your secure password.
3. Access: Your access is linked to the patient's record. You will only see what the patient has authorized (e.g., appointment schedules, progress notes).
Breathwork / Wellness User
You are registered for a standalone Breathwork or general Wellness program.
1. Username: Enter the Email Address used for course enrollment.
2. Password: Use the password you created during checkout/program signup.
3. Need Help? If you cannot remember your password, use the "Forgot Password" link.
Clinical Staff / Provider
You are a healthcare professional, therapist, or administrator.
1. User ID: Enter your assigned Staff ID (e.g., JSmith123) or your registered work email.
2. Password: Enter your secure credential.
3. Security: Clinical access requires Multi-Factor Authentication (MFA). You will be prompted to enter a unique code sent to your registered work phone/authenticator app.
